Note : Visa is handled for Himachal Pradesh, Uttaranchal, Uttar Pradesh, Madhya Pradesh, Jammu and Kashmir, Punjab, Haryana, Delhi, Rajasthan, Bihar, Chattisgarh, Jharkhand, West Bengal, Odisha, Sikkim, Assam, Tripura, Mizoram, Arunacal Pradesh, Nagaland, Meghalaya, Manipur and stateless persons only.
Visa Requirements
TOURIST VISA FOR POLAND
Passport should be valid for at least three months beyond the intended stay in the Schengen area. It must have minimum two blank pages and must not be older than 10 years. All schngen visa copies of Old & new passport.

Two recent color photographs, 35 x 45mm in size with white background (matt finish) and the face should cover 80% of the photo area
Covering letter from the applicant stating name, passport details, purpose and duration of stay
Proof of airline reservation e.g. reservation slip
Day to day itinerary Overseas medical insurance valid for the country with the minimum coverage of 30000 Euro. The insurance has to cover all risk e.g. accident, illness e.t.c.The policy must be clearly specify the period of validity and has to cover the date of arrival and departure and has to be submitted in original along with photocopy
A no objection certificate from the current employer
Proof of accommodation: hotel reservation including the name, email id and phone number of hotel
Proof of sufficient funds Bank statement for last 3 months including address and telephone number of the bank, A/C holder’s name & address with stamp and signature of bank
Income tax returns for last 2 year
3 months salary slips
If you are self employed: registration of the company/ partnership deed/ Memorandum of Article (MOA) is required
Two copies of passport first and last page to be submitted
Please note for NEW DELHI submission if the passenger has never been to schengen country before then he has to come personally to apply Tourist visa
Applicants should not purchase their tickets before their visa is approved where proof of tickets issued is not listed in the checklist as a mandatory document to be provided with the application. Neither the Consulate nor Ziyarat e Harmain takes any responsibility for any ticket purchased by the applicant without a visa being issued NOTE: A short stay Schengen visa can be allowed to enter and stay in the Schengen area for a maximum of 90 consecutive days or 90 nonconsecutive days (several trips) out of a period of 180 days, from the date of the first entry in the Schengen area. Poland Consulate will not accept any application if the travel date is not beyond 10 working days of submission. Delhi : The authority letter from applicant should be hand written for Poland submission.
BUSINESS VISA FOR POLAND

Invitation letter from the Company of Poland and applicant’s name and passport details, Email ID should be mentioned and should be directly faxed to the Embassy or the Consulate with one copy to the applicant
