TOURIST VISA FOR SLOVAKIA

 

 Passport should be valid for at least 3 months from the date of submission of the visa application

 

 One Visa Application form to be filled completely and signed by the applicant.

 

 One recent color photograph, 35x45 mm in size with white background.

 

 Covering letter from the applicant stating his name, purpose and duration of visit

 

Proof of airline reservation for a round trip e.g. reservation slip

 

 Original hotel voucher with confirmation of payment for stay in Slovakia.

 

 Conditions of Voucher – Name and address of travel agency, registration number of voucher, first and last name of hotel guest, date of arrival and departure, description of hotel services, confirmation of the authorized person of the travel agency. If the voucher does not include the confirmation of payment, applicant has to submit separate invoice with the number, stamp and signature of bank statement confirming the payment on the account of hotel/agency

 

 Overseas medical insurance valid for the country with the minimum coverage of 30000 Euro. The insurance has to cover all risk e.g. accident, illness e.t.c.The policy must be clearly specify the period of validity and has to cover the date of arrival and departure and has to be submitted in original along with photocopy

 

 In case, applicant is being invited by a person in Slovakia, then:

 

 Official invitation verified by the Slovak Foreign Police Department

 

 Original invitation letter of guarantee for all expenses from the person in Slovakia legalized by a notary public

 

 Proof of sufficient funds

 

 Bank statement for last 6 months including address and telephone number of the bank, A/C holder’s name, address with stamp & signature of bank

 

 Income tax returns for last 3 year

 

 6 months salary slips

 

 Two copies of passport first and last page to be submitted.

 

 Applicant needs to appear for the personal interview

 

 Old passport in original required

 Applicants should not purchase their tickets before their visa is approved where proof of tickets issued is not listed in the checklist as a mandatory document to be provided with the application.

 

Neither Consulate nor Ziyarat e Haramain takes any responsibility for any ticket purchased by the applicant without a visa being issued.

 

BUSINESS VISA FOR SLOVAKIA

 

 Passport should be valid for at least 3 months from date of submission of the visa application

 

 One Visa Application Form to be filled completely and signed by the applicant

 

 One recent color photograph, 35x45 mm in size with white background

 

 Covering letter from the company stating the purpose and duration of stay

 

 Proof of current occupation i.e. letter from the employer

 

 Proof of airline reservation for a round trip e.g. confirmed air ticket

 

 Proof of accommodation.

 

 Original Invitation letter from the Slovakian Company verified by the Slovak Foreign Police department it should contain Details of the inviting person – name, address, phone number, Date and place, where the letter was written, Details of invited person – name, address, phone number, The invitation letter should be signed by the inviting person.

 

 Letter of guarantee from the Slovakian company which must specify that company will be financially responsible for the applicant and should be legalized by the notary public
